Planning Units shall notify Cheryl Sickels, Position Information Manager of changes to a position that could impact the position number. Complete the Position Modification form along with any necessary supporting documents. In some instances, it may be necessary to use this form as a supplement to other documents such as a PDQ or JIQ.
A Position Modification form may be used to do a combination of the following actions:
Status of a position (full-time to part-time, etc.)
FTE change
Title change of a position (work through HR-Compensation Office)
Number of months worked has changed
Budget organization change
Supervisory change
Abolish a position
Funding source change
Request a position audit
