Benefits Open Enrollment 2009

The deadline for submitting open enrollment forms was May 15, 2009. If you haven't already returned your form please do so as soon as possible to ensure correct benefits coverage for yourself and your family.

**CORRECTION**
The PPO coverage chart included in the enrollment packets contained an error. The chart should indicate that inpatient and outpatient services and surgery, and inpatient mental health are subject to the deductible. The corrected chart may be downloaded below. This error was reported in the 5/6/09 edition of Outlook.  

Each year employees have the opportunity to make changes to their benefit elections during the annual open enrollment period. Otherwise, changes may only be made if a family status change has occurred.

Benefits Open Enrollment formIndividual statement detailing your current coverage; changes for the upcoming plan year may be made by selecting the appropriate box in the column to the right. Forms must be signed and returned even if you are not making changes.
NOTE: If covering a spouse or partner be sure to answer the questions above the list of covered dependents before returning your form!!!

  • Child Eligibility Extended to Age 25 with $50 Monthly Premium
  • Domestic Partner Eligibility Expanded to Opposite Sex Partners
  • Legal Guardians Now Eligible for Health Benefits

Prescription Benefit Management Company to Change July 1, 2009
Effective July 1, 2009 the University's prescription drug plan, including the mail order option, will be administered by Express Scripts.
