If you are separating employment with Ohio University, you'll want to make sure you have taken care of all necessary details.

 
Employee

  • Written notice to Supervisor

  • Return library materials

  • Close Bobcat Cash Acct

  • To continue insurance contact UHR about COBRA

  • Turn in to supervisor:

  • Parking Tag

  • Purchasing Card

  • Building, Room, Car Keys

  • University electronic equip

  • Bobcat Calling Card

  • University ID

  • Dept or Univ Tools

  • Retirees Only:  Sick Leave Conversion form

  • Contact the Payroll Department with questions regarding vacation payout, etc.
Supervisor

  • Notice to UHR Records

  • Collect dept resources

  • Update dept website

  • Paid Time Off forms submitted to Payroll timely

  • Limit IT systems access
