preventive care
The following preventive care procedures are covered under each health plan as indicated in the coverage chart. The procedures listed below are the only covered preventive care benefits.
- Routine Physical Exam: 1 every 2 years for ages 9 and over
- Tetanus Vaccine: 1 every 10 years for ages 9 and over, office visit not included
- Hepatitis B Vaccine: is covered once for ages 12-16, unlimited for 16 and older
- HIV Screening: 1 screening every 2 years
- Total Cholesterol Screening
- Prostate Specific Antigen and Digital Rectal Exam
- Pneumococcal: Once for ages 65 and over
- Electrocardiograph Stress Test: associated visit (reading) included
- Thyroid Panel: office visit not included
- Fasting Glucose: 1 every year for high risk individuals (i.e. those who are obese and/or have a family history of diabetes); office visit included
- Sigmoidoscopy
- Mammograms
- Pap Smears
- Venipuncture
- Child Health Supervision (Well Child Care): Total plan benefits of $2400 from birth up to 2 years of age, $150 for each age interval afterward is listed: 2-3 years, 3-4 years, 4-5 years, 5-6 years, 6-7 years, 7-8 years, 8-9 years
- Colonoscopy: 1 every 5 years for ages 50 and older
