| 1. | Immediately contact GE Corporate Payment Services Lost/Stolen Card Service, available 24 hours a day: |
| | Toll-free at 1.800.274.7378 -OR- When traveling internationally, call collect 1.801.464.3232 |
| 2. | Contact the Purchasing Card Administration at 740.597.OHIO (6446), so your PCard account profile can be re-established, and a new replacement plastic can be ordered. |
| 3. | If it is suspected that the card has been stolen while on Ohio University property, notify Campus Safety. |
| 4. | A replacement card will be reissued upon report of the lost or stolen card to GE Corporate Payment Services. (NO PAPERWORK REQUIRED). This may take 7-10 business days and will be delivered to the Pcard Administration. |
| 5. | Arrange for alternate procurement arrangements (as the "replacement" card may take 7-10 business days). |
| 6. | Upon receipt of the "replacement" PCard, be sure to activate the new account before initiating use. |
| 7. | Notify your PARIS reviewer of your last valid transaction. |
| 8. | The PARIS reviewer must continue to monitor the lost card account on PARIS until all transactions have been reconciled. |
| 9. | If the PARIS reviewer is the cardholder, a new user ID and password, specific to the replacement card will be assigned. |
| 10. | If a lost/stolen PCard is later found, destroy the card immediately. The card is not functional after being flagged stolen or a new replacement card has been issued. |
