New and Revised Forms: I-9, Payroll Expense Accounting Correction (PEDS), Ohio University Appointment, Parttime New Hire Checklist
The payroll department is in the process of revising our forms. As new and revised forms become available, they will be listed at the top of the Payroll News section. All forms will be available on the downloadable forms page (link located on the left side of this page) unless otherwise noted. Please discontinue using the old forms immediately.
Reappointment will be open from Wednesday, May 1 until midnight Sunday, June 2. Payroll has scheduled open labs on the following dates and times. Payroll staff will be available to assist you with questions regarding the reappointment process.

Separations and Terminations
Please notify Claire Carson email@example.com or Bridget Driggs firstname.lastname@example.org in the Payroll Office immediately of any salaried appointments (administrative staff or faculty) who have resigned, retired or have been terminated. Also, notification of any employee who should not receive a full pay on the next Semi-monthly pay cycle due to unpaid leave of absence, fellowship faculty leave, insufficient paid time off accruals, etc.
