Education Abroad Forms
Please read each description carefully to determine which form is needed for your particular experience. If you have questions, please call 740-593-4583.
Ohio University Study Abroad/Exchange
If you are applying to the Puerto Rico program or the Global Theater Institute, please apply through our pilot application program online at http://eais.admsrv.ohio.edu/eais/student/.
If you plan to go on an Ohio University study abroad or exchange program you will fill out the general application packet to begin.
Once you have been accepted to the program, please submit the following forms:
*Please come to the Office of Education Abroad to obtain the Foreign Study Checklist.

Non-OU Program-for credit
If you are participating in a program not sponsored of Ohio University and you will be receiving transfer credit for your studies, you will need:
- Foreign Study Checklist*
- A copy of the photo page of your passport
- Flight Itinerary

Independent study, research trip, or other OHIO-affliciated travel-not for credit
If you are doing an independent study or research abroad outside of an organized program, you will need:
