Community Outreach
Outreach Mission: To provide diabetes-related education and community services to the Appalachian community.
Outreach Emphasis:
- Provide free diabetes clinics
- Provide community screening clinics
- Assist uninsured and low income access resources to monitor and control disease
- Assess and identify outreach needs
- Evaluate success of outreach activity and revise programs as needed
- Provision of continuing education
