Examples of Completed Performance Management Review Forms

Below are examples of completed Performance Management Review forms. Use these as a reference when completing forms for the performance management program.

Using the Performance Review Form:
Part I: Accomplishments
After reviewing the Performance Plan, and the documentation you have kept on the employee for this performance period, draft a description of the employee's accomplishments. In your performance management review meeting, ask the employee to share their notes on their accomplishments. Revise or add to your description as needed.

Part II: Ohio University Competencies
Revisit the Performance Planning Form (#PM2) and the Mid-Year Review Form (#PM6). Determine what level you will rate the employee for each of the University's competencies. Have specific examples prepared.

Part III: Overall Performance
Carefully consider the overall performance rating.

Part IV: Goals
Carefully review the employee's area for growth and consider goals you would like to see established for the next performance period. The employee should be prepared with the completed
Employee Self-Evaluation Form (#PM5).
