During the Performance Planning Session, the initial stage of the Performance Management process, supervisors are expected to meet with their employees and complete the Performance Planning Form (#PM2.) At this time they need to decide whether or not they will use the Supervisor Feedback Form. Following the employee’s Year End Review, the form is to be shared with the supervisor prior to submission to his or her supervisor.

Note to Employees
The revised form is based on the same seven competencies currently used to evaluate classified and IT employees. Supervisors should be evaluated based on how the competencies apply to their supervision of you. Please consider using this form to clarify expectations and goals with your supervisor, and to help establish open two-way dialogue.

Note to Supervisors
As a leader, you influence employees, guiding and providing direction when needed. Through on-going coaching, you are able to enhance the performance of employees while maintaining effective communication. You demonstrate by example, your expectations and best practices. Finally, your role as a motivator and supporter is critical to creating an inviting yet challenging environment in which to work and produce successful results. Please encourage your employees to give you the feedback you need to help develop you in your role as a coach, mentor and supervisor.

We hope the revised form will promote partnership and mutual growth.
