Application Requirements:
1. Cover Sheet/Recommendation Form

4. Chair's Plan for Departmental Coverage of Teaching Load and Responsibilities (click for details)
5. Letters of Support - Separate letters of support from each person signing the "Recomendation for Faculty Fellowship Leave" form must be included with the proposal and should address the merits of the proposal and the quality of the faculty member's teaching and research or creative and scholarly activity.
6. Curriculum Vitae - The applicant must include an updated CV with the proposal.
NOTE!
A short one to two page report on the completed University Fellowship leave shall be submitted in writing to the department within three months following the return of the faculty member to his/her teaching duties at Ohio University. The department will evaluate the report and forward it to the dean who will review it and forward it to the Provost. Evaluation of this report will play a significant role in the granting of future University Fellowships. Not submitting a report may negatively impact the approval of future leaves.
